New! United Blood Services can now accept a Fast Track Ticket from some mobile devices (smartphones or tablets) as long as their staff can scan the barcode from the ticket. Simply complete the health history questionnaire on your mobile device and bring the device with you when you donate. Or, you can still complete the questionnaire on your computer, then print and bring the Fast Track Ticket with you.

Important: Before you start the questionnaire, please enter your first and last name exactly as it appears on your donor ID card (if you don’t have a card, use your legal name). Do not use punctuation marks (such as apostrophes) or special characters (such as hyphens).
